Masters requires a valid Certificate of Insurance for General Liability and Worker's Compensation, prior to commencement. Please download and review the Sample Certificate below for required wording and applicable limits. (Please note that client-specific certificate wording may be required on some projects.)

If you are Worker's Compensation Exempt, please download and complete the Affidavit below and submit it to our office with a photocopy of your valid WC Exemption Card:
